Pharmacological / Clinical Information

Indications / Uses: Tiromel is used to treat hypothyroidism (insufficient thyroid hormone), to replace or supplement thyroid hormone where needed. 

Mechanism / Action: It provides the T3 hormone (liothyronine), which is the more active thyroid hormone form, thereby raising T3 levels and increasing metabolic rate in tissues. 

Dosing / Regimen Guidance:

  • For mild hypothyroidism, the recommended starting dose is 25 µg per day; the dose can be increased by 12.5–25 µg every 1 or 2 weeks. 

  • Usual maintenance doses are in ranges such as 25–75 µg per day (depending on severity) 

  • In more severe cases (e.g. myxedema), starting doses may be lower (e.g. 5 µg) and gradually titrated upward. 

  • Dosing must be individualized according to patient’s response, lab values (TSH, free T3/T4), and tolerance. 

Pharmacokinetics / Properties:

  • Being T3, it is more rapidly acting than T4 (levothyroxine) and has a shorter half-life. (General thyroid pharmacology)

  • It is synthetic but identical to the endogenous T3 in action
